Use of Website
Each person assumes full responsibility and all risks arising from use of this site. Illegal use or abuse of any self-propagating programs on this site will be subject to deletion of entries with no refunds (if applicable) for unused time should there be an offer of posting information for selected or advertised time periods. Anyone disregarding posted legal notices or cease and desist requests will be subject to full legal action as allowed by city, county, state and/or federal laws.
PRISM Body does not knowingly collect personally identifiable information from children under the age of eighteen. If you are under the age of eighteen, you must ask your parent or guardian for permission to use this website.
To schedule and receive PRISM Body’s services, all users under the age of 18 are required to have a legal guardian create an account, sign a Minor Release Form, and be present in the office for the duration of the Service.
Online Conduct
Members and visitors agree to use this Site only for lawful purposes and are prohibited from (1) posting any unlawful, harmful, threatening, abusive, harassing, defamatory, obscene or otherwise objectionable material of any kind on this site; (2) attempting to obtain unauthorized access to the Site or another’s account; (3) interfering with the proper working of the Site including, but not limited to, transmission of any virus, worm, trap door, timer, or other limiting routine, instruction or design; (4) impersonating any other person or entity, whether real or fictitious. PRISM Body may terminate your access to the Site at any time for violation of the Terms of Use.
Personal Information
Scheduling features (Acuity) of the Site will require the collection of personal information required to provide the Service. If you are required to register with Acuity and select a unique login and password (“Personal Login Information”), you must keep your Personal Login Information confidential. Your Personal Login Information is personal to you, and you may not allow any third party to use it.
Limitation of Liability
PRISM BODY IS NOT LIABLE OR RESPONSIBLE FOR ANY CLAIM, LOSS OR DAMAGE OF ANY KIND, WHETHER DIRECT OR INDIRECT, IN CONNECTION WITH OR ARISING FROM YOUR USE OF, OR INABILITY TO USE, THE PRISM BODY WEBSITE. In the event you are dissatisfied with, or dispute, these Terms of Use and the Site or content, your sole right and exclusive remedy is to terminate your use of the Site.
Complete Agreement
These Terms of Use and your use of the Site are governed by the federal laws of the United States of America and the laws of the State of Colorado, without regard to conflict of law provisions.
These Terms of Use are the complete agreement of the parties with respect to use of the Site. If any provision is held by a court or other tribunal of competent jurisdiction to be unenforceable, then such provision(s) shall be limited or eliminated to the minimum extent necessary so that this agreement otherwise remains in full force and effect. Terms & Conditions for receiving services
“It is my choice to receive massage therapy. I am aware of the benefits and risks of massage and give my consent for massage. I understand that there is no implied or stated guarantee of success of effectiveness of individual techniques or series of appointments. I acknowledge that massage therapy is not a substitute for medical care, medical examination or diagnosis.
I have stated all medical conditions that I am aware of and will inform my practitioner of any changes in my health status.
I understand that my personal health information will be collected. I understand that all information that I provide will be kept confidential unless required by law. I understand and consent that my medical information may be shared by the various care providers involved in my care and treatment.
Treatments may be covered by extended health care plans. I understand that it is my responsibility to confirm the exact details of my coverage. I understand and consent to the use of my email address for communications about our services.”
